From time to time, students may need to submit an application and/or student form related to their course or enrolment details.  To get started please choose from a category below, or if you can't see the form you need please email the Student Support team via: This email address is being protected from spambots. Each application or student form will have specific information on how, and where to submit the documentation: 

EXPLAINED ABSENCE FORM
For in-class absences. Forms must be submitted by the due date with the appropriate supporting documentation attached.

PROFESSIONAL PRACTITIONER MEDICAL CERTIFICATE
For in-class absences. Forms must be submitted by the due date with the appropriate supporting documentation attached.

APPLICATION FOR EXTENSION
For rescheduling assessments and exams. Supporting documentation must be submitted. Extensions are approved by the Head of Department.

SPECIAL CONSIDERATION
For students that have been adversely affected by a serious and unavoidable disruption to their studies beyond their control.

ACADEMIC APPEAL OF GRADE
If you believe your final grade in a unit does not accurately reflect your performance against the unit outcomes, and have been unable to resolve it with your Head of Department.

ACADEMIC TRANSCRIPT REQUEST FORM
An academic transcript is a formal record that lists the units and course(s) you have completed at AMPA.

REPLACEMENT TESTAMUR FORM
A testamur is the legal certification of your degree. You can only request a replacement testamur if the original testamur has been lost, stolen or damaged (a Statutory Declaration will be required) or you have legally changed your name since your testamur was issued.

Pathways to AMPA
 



Have you completed an RTO Diploma in Dance, Music or Music Theatre? You may be eligible to receive credit for units completed when upgrading your qualification to an AMPA Bachelor degree.

The Pathways Program provides graduates of endorsed RTO courses with credit towards the AMPA Bachelor of Dance or Bachelor of Music (Perf.). Therefore, if you have completed a recognised full-time course you could be eligible to receive:

* up to 50% credit (or 3 trimesters) for an endorsed Advanced Diploma
* up to 33% credit (or 2 trimesters) for an endorsed Diploma

The above figures are indicative and based on the maximum attainable credit granted. The number of units credited will depend on the course you have completed, the provider and endorsement status with AMPA.

As AMPA is an approved Fee-Help provider you will have the option of deferring your fees through the Study Assist program.

The Academy of Music And Performing Arts (AMPA) is one of Australia's leading tertiary providers of performing arts education.

Educating and inspiring young artists for over 30 years, AMPA remains committed to providing the highest quality in music and performing arts degrees, including music production and composition, contemporary and classical music, music theatre and dance performance (contemporary, ballet and jazz).

With its distinctive personal approach, high calibre teaching staff, emphasis on performance-based study and individual study designs, AMPA equips talented artists with the skills essential to succeed as musicians, dancers, performing artists, composers, producers, choreographers, directors, teachers and industry professionals in Australia and around the world. Recent Developments

Recent developments at AMPA include:

  • Rated #1 by students for positive interactions with staff and students amongst similar tertiary providers in NSW (SES Survey 2021 & 2022). 
  • Expansion of individual music units to ensure AMPA students receive more individual lessons and mentoring than any other tertiary provider in Australia. 
  • Expansion of postgraduate courses to 18 weeks per study period (including 18 x individual Principal Study & Second Study units).
  • The scheduling of major external dance showcase events for the end of every trimester.
  • The introduction of guest choreographers to work with students each trimester. 
  • Campus upgrades to the Tom Mann Theatre, ensemble studio, recording/production studio and tracking rooms.
  • Establishment of the Electronic Digital Instrument (EDI) and Video Games Music (VGM) as a Principal Study, allowing for 12 x individual mentoring sessions with a specialist.
